STAFFING OF PRIMARY HEALTH CARE FOR THE ADULT POPULATION OF THE RUSSIAN FEDERATION IN 2014-2022. PART 1: FOCUS ON DISTRICT INTERNISTS

Summary:
Introduction (relevance). Issues of accessibility, quality, effectiveness of medical care, satisfaction of the population with the results of its provision cannot be solved without the availability of qualified personnel in sufficient numbers. The purpose is to analyze the indicators of staffing of primary health care and their dynamics for the period 2014-2022 in terms of district internists. Materials and methods: analysis of data on the number of individuals of district internists in 2014-2022, the security of the adult population and qualification categories according to the data presented in the statistical collections of the Federal State Budgetary Institution "TSNIIOIZ" of the Ministry of Health of the Russian Federation. The method of descriptive statistics is used. Results. It shows a decrease in 2014-2016 and a subsequent increase since 2017 in the indicators of the number of district internists and their provision for the adult population as a whole in the Russian Federation, even during the period of a new coronavirus infection (2020-2021). At the same time, there is a pronounced unevenness of indicators and their dynamics between the subjects of the Russian Federation. The proportion of district internists who had qualification categories decreased throughout the study period. Discussion. The nature of the change in the number of district internists and the security of the population as a whole in Russia indicates, first of all, the positive effect of the transition from 2017 to a new system of admission to the professional activities of university graduates in the specialty "medical business". Changes in the subjects of the Russian Federation have a multidirectional nature of changes, but in many of them primary medical care for the adult population is provided not only by district internists, but also by general practitioners. In addition, the practice of expanding the areas of responsibility of secondary and non-medical personnel can also have an impact on the staffing indicators of primary medical care. The distribution of qualification categories was influenced by several factors. All of the above determines the expediency of further analysis of similar data on general practitioners, as well as sociological research, including in terms of qualification categories. Conclusions. The period 2014-2022 was characterized by a decrease in the number of district internists in 2014-2016 and its constant increase during 2017-2022, the growth rate was 10.10%. The growth rate of the security indicator in 2017-2022 amounted to 10.37% in Russia as a whole. Between the federal districts, the indicators of the number of district internists differ by more than 4 times, between the subjects of the Russian Federation - by more than 200 times, the differences in security were 1.5 times and 4.3 times, respectively. During the study period, there was a decrease in the proportion of district internists with a qualification category, both in general and for each of the categories separately.
