STAFFING OF PRIMARY HEALTH CARE FOR THE ADULT POPULATION OF THE RUSSIAN FEDERATION IN 2014-2022. PART 3: FOCUS ON PRIMARY CARE PHYSICIANS

Introduction (relevance). The article presents a continuation of the study devoted to the analysis of the staffing of primary medical care in the Russian Federation for 2014-2022. Since primary medical care for the adult population in the Russian Federation is provided by district internists (VTU) or general practitioners (GP), for the most complete characteristics of the provision of the adult population with doctors providing primary medical care, it is advisable to analyze the total indicators of the number and provision of VTU and GP, as well as to assess the dynamics of these indicators. The purpose is to analyze the total indicators of the number of doctors providing primary medical care to the adult population of the Russian Federation, security and their dynamics in the period 2014-2022. Materials and methods: the data sources were the statistical collections of the FSBI "TSNIIOIZ" of the Ministry of Health of the Russian Federation for 2014-2022. The method of descriptive statistics is used. Results. The share of VTU in the total number of doctors providing primary medical care throughout the study period was 78% in 2014, 75% in 2018 (the smallest share), and 81% in 2022. The total provision of the adult population with doctors providing primary medical care in the Russian Federation as a whole amounted to 4.13 per 10,000 in 2022, despite the fact that the provision of VTU and GP amounted to 3.3 per 10,000 adults and 0.62 per 10,000 of the total population, respectively. A more favorable dynamics of security indicators in the studied period was also obtained. The differences in the total provision of the adult population with primary care doctors between the maximum and minimum values in the Russian Federation amounted to 3.3 times, in federal districts – from 3.3 (Central Federal District) to 2.00 (North-Western Federal District). The subjects of the Russian Federation are grouped according to the levels of provision of the population with doctors providing primary medical care and its dynamics. Discussion. The analysis of the total indicators made it possible to significantly clarify the characteristics of the staffing of the adult population with primary medical care: an increase in the total provision of the adult population of VTU+VOP by 25% compared to the VTU security indicator. This, in turn, created prerequisites for forward-looking assessments and further development of methods for evaluating the effectiveness of personnel work in the subjects of the Russian Federation and the selection of best practices. Conclusions. In general, in Russia to date, the number of VTU significantly exceeds the number of GPOs. Taking into account the GP allowed us to clarify the indicators of the provision of primary medical care to the adult population. A positive aspect is the reduction of differences in the indicators of the subjects of the Russian Federation. Thus, it is advisable to evaluate the total indicators of staffing of primary medical care for the adult population.
