FEATURES OF COMMITMENT TO A COMBINED REHABILITATION PROGRAM AMONG PATIENTS WITH COGNITIVE DISORDERS

Summary:
The problem of low adherence to the recommended line of therapy among patients with cognitive impairment in modern gerontology comes to the forefront, making it difficult to treat the underlying disease and thus having a negative impact on the patient's individual viability. The aim of the study is to assess the commitment to a combined rehabilitation program among patients with mild cognitive impairment. Materials and methods. The study included 203 patients between the ages of 45 and 75 (of whom, 117 women, 86 men) with mild cognitive impairment. In accordance with age characteristics, patients were recommended a combined rehabilitation program with the inclusion of a line of drug therapy, a course of individual physical activity and an additional unit in the main group patients - an algorithm of actions (certificate of state registration of the program for computer No.2019614243) as a way to correct cognitive defect. Levels of physical activity are randomized by age characteristics of patients with a gradual increase in the duration of exercise. The duration of the rehabilitation program was 6 months. The commitment to the rehabilitation programme was assessed for 3 and 6 months of the study. The study materials are statistically processed using IBM SPSS Statistics v.23. The test of the validity of the results was carried out using the Student criterion. The results of the study were considered reliable at p.05 (at 5% level of significance). Results and their discussion. In evaluating the results of the study among patients with cognitive impairment who received a combined rehabilitation program, adherence to the program is reliably higher in the main group, both among elderly and middle-aged patients. Among elderly patients in the control and main groups, a reliable difference was established in 6 months between the number of patients who exercise regularly (p=0,028) and the true difference in the number of those who did not exercise proposed physical activity program (p=0,026). Also among middle-aged patients in the control and main groups, there is a reliable difference in the number of patients regularly taking drugs during cognitive rehabilitation of 6 months (p=0,001) and irregularly taking drugs for cognitive rehabilitation of 6 months (p=0,002). Conclusions. The results of the study demonstrate the importance of correcting cognitive impairment in patients whose recommendations include combination therapies as a key component of increasing adherence to line therapy. In identifying the reasons for the decline in commitment, a set of non-cognitive causes has been identified, which should be emphasized in the appointment of combination therapy and rehabilitation regimens
