Summary:
Introduction. Lesions of peripheral vessels, against the background of the spread of atherosclerosis, comorbidity of diseases of the circulatory system, become a significant medical and social problem. Complications requiring surgical treatment are associated with severe health consequences, disability, and significant resource costs. The purpose of the study is to identify the features of the organization of specialized medical care for patients with peripheral artery diseases using surgical methods of treatment in St. Petersburg – a city with a significant proportion of the population of older age groups. Material and Methods. The data of 25 hospitals in St. Petersburg for 2019 on the provision of specialized medical care for diseases of the arteries of the lower extremities in the profile "cardiovascular surgery" were studied: by a solid method according to form No. 14 "Information on the activities of units of a medical organization providing medical care in stationary conditions"; by a selective method according to primary medical documentation "Medical card inpatient patient" 1650 patients. The information was copied onto a specially designed map, statistically processed with the calculation of intensive and extensive indicators, average values (M ± δ) with a risk of 0.05 error, evaluation of the significance of differences according to the Student's t-criterion. The initial data had a normal distribution. Results. Less than a quarter of cases are reconstructive surgical interventions. In every tenth case, patients undergo additional diagnostic and therapeutic procedures, which increases the waiting time for surgical treatment to two weeks. In the discharge recommendations, only in 30.4% of cases in patients who underwent surgical treatment, the need for further dispensary follow-up is indicated. Discussion. Contrary to the recommendations, reconstructive operations are not a priority in the provision of specialized medical care, which indicates the lack of control over the timeliness of treatment of patients at the prehospital level, aimed at preventing complications, saving limbs. Conclusion. The introduction of a register of patients with peripheral artery diseases to level out defects in their medical care should be the basis for management decisions to improve the quality and accessibility of medical care.
