IMPROVING THE PROVISION OF MEDICAL CARE TO THE POPULATION IN THE FIELD OF "ENDOCRINOLOGY" AT THE REGIONAL LEVEL

Introduction. The existing system of providing medical care in the field of endocrinology requires improvement to improve its quality and accessibility in areas with a vast geographical extent and a high proportion of the rural population. The presence of only specialized endocrinological centers in large cities of such regions experiences a high flow of patients from all over the region, and also determines the insufficient level of accessibility of such care to the population in areas remote from such centers. In this regard, it becomes obvious that there is a need to create interregional endocrinological centers to solve existing problems of providing medical care in the field of "endocrinology". The aim is to scientifically substantiate recommendations for improving the organization of medical care for patients with endocrine diseases at the regional level. Materials and methods. To assess the dynamics and territorial characteristics of the incidence of endocrine diseases in the adult population, a statistical analysis of the data of the GBUZ "Medical Information and Analytical Center" on the incidence of the adult population and the staff of endocrinologists, as well as a hierarchical cluster to assess the territorial characteristics of the data studied, was carried out. A sociological survey of 1,200 adult patients living in areas planned to be serviced by inter-district endocrinological centers was conducted using a questionnaire method based on the questionnaire of N.V. Yurgel et al. (2009). Results. The overall incidence of endocrine diseases at the regional level tended to increase. The primary incidence of had a statistically pronounced trend. The territories of the region can be divided into three clusters depending on the levels and dynamics of morbidity. Differences between clusters may be due to differences in disease diagnosis levels and access to medical care. The staffing of endocrinologists over the analyzed period of time tended to decrease at the regional level. Clusters with stable staffing levels, growing and decreasing, have been identified. Despite the positive dynamics of the part-time ratio, there are territories where the rates of endocrinologists are closed at the expense of doctors working at their main place of work in other positions. Satisfaction with the quality and accessibility of medical care for the population of the districts planned to be serviced in the inter-district endocrinological centers being created is almost the same, however, more pronounced staffing and organizational problems have been identified in the East of the region. Conclusion. The main way to increase the availability of medical care in the current conditions is the creation of inter–district endocrinological centers with the retention of positions of endocrinologists in district hospitals
