SOCIOLOGICAL STUDY OF FACTORS INFLUENCING THE OUTFLOW OF DISTRICT PHYSICIANS FROM MEDICAL ORGANIZATIONS OF THE PUBLIC SECTOR OF REGIONAL HEALTHCARE

Summary:
Introduction. Scientific studies devoted to the analysis of the current situation and assessment of prospects provide data characterizing the factors that have the greatest impact on the outflow of qualified personnel, which include the difficulties of professional activity associated with the specifics of work, «professional burnout», shortcomings determined by problems of long-term personnel planning and professional development. In the long term, experts predict a deterioration in the provision of medical service density. In view of this, it seems relevant to study the factors influencing the outflow of district physicians from medical organizations in the public sector of regional healthcare.
Purpose of the investigation. Analysis of factors influencing the outflow of general practitioners from medical organizations of the state sector of regional healthcare.
Materials and methods. The study was conducted in 2024 and was selective. The objectives of the sociological observation were to study individual unfavorable factors of professional activity and to determine the factors influencing the reduction in the number of district general practitioners in medical organizations of the regional healthcare system. The methods of observation, comparison, descriptive statistics, and linear regression analysis were used.
Results and discussion. Interpretation of the sociological observation results showed that 75,7% of respondents are at risk of professional burnout. Among the unfavorable factors of professional activity, the leading positions are occupied by: discrepancy between the salary level and the amount of workload on a district general practitioner (1st rank place); large volume of medical documentation (2nd rank place) and high level of responsibility (3rd rank place). The leading factors increasing the risk of outflow of district general practitioners from the profession have been established: «decrease in the prestige of the profession» (response frequency of 73,7 out of 100 respondents), poor work organization (49,5 out of 100 respondents) and irregular working hours (35,1 out of 100 respondents). The results of the regression analysis prove a high degree of dependence between the unfavorable factors of professional activity and the risks of leaving the profession or the risks of moving to the non-state healthcare sector for district general practitioners, confirmed by the value of the multiple correlation coefficient (R2=0,968017); the degree of closeness of approximation of the data of the selected function (R2=0,937056); low value of the standard error of approximation (0,059370); the value of the Fisher criterion (74,43565) at a significance level of 0,000345.
Conclusion. The main factors influencing the outflow of district physicians from medical organizations of the state sector of regional healthcare include the discrepancy between the level of salary and the volume of work performed; large volume of documentation; large volume of functional responsibilities; high level of responsibility; unfavorable psycho-emotional climate; high static load, confirmed by the high level of reliability of the regression model.
