STAFFING OF MEDICAL ORGANIZATIONS OF THE HEALTHCARE SYSTEM WITH OTORHINOLARYNGOLOGISTS (LITERATURE REVIEW)

Summary:
Introduction. One of the important conditions for providing high-quality and timely medical care to patients is the availability of a sufficient number of qualified medical personnel. However, a common problem for modern healthcare systems in the Russian Federation and several developed countries is the shortage of medical personnel and, as a result, the high workload on medical staff, which can lead to medical errors and burnout. Given that ENT pathology accounts for a significant share of the overall morbidity rate and is a significant socioeconomic health issue, studying and analyzing data on the current challenges of staffing medical organizations in the healthcare system with otolaryngologists can be crucial in finding solutions aimed at attracting and motivating medical professionals in the field of otolaryngology.
Purpose: conducting an analysis of the staffing issues in healthcare organizations and studying their relevance in otorhinolaryngology, as well as identifying their causes and finding effective solutions.
Materials and methods. Domestic and foreign literature was analyzed in the following databases: CENTRAL, MEDLINE, PubMed, and EMBASE, eLibrarу.ru, cyberleninka.ru, scholar.google.ru, lib.rudn.ru, legislative and regulatory documents, and the search portal of the International Register of Clinical Trials of Randomized Controlled Studies dedicated to the issue of staffing medical organizations of the healthcare system with otorhinolaryngologists. The search was conducted using the following keywords: "review", "staffing", "otorhinolaryngologist", "ENT organs", "healthcare system", and "otorhinolaryngological assistance". More than 200 sources were studied over a period of 15 years, and 42 of them were included in this review. The following methods were used: analytical, content analysis, bibliographic, and logical.
Results. According to a number of studies conducted by domestic and foreign authors, there is a persistent shortage of medical personnel in the global healthcare system. According to various estimates, this trend will continue, and the shortage of medical staff will increase: by 2030, there will be a global shortage of medical staff of approximately 10 million, and by 2035, it will reach 12.9 million. According to the Ministry of Health of the Russian Federation (RF), the number of specialists (doctors, mid-level and junior medical staff) working in medical organizations has decreased by 217.8 thousand compared to 2023. As a result of the analysis of the state (passportization of regions) conducted in 2018, it was revealed that there is a shortage of otorhinolaryngologists by 14.2%. This trend is observed in 63 regions of the Russian Federation. Over the period 2018-2023, there was a decrease in the number of ENT specialists working in state-owned medical organizations (MOs) by 122 specialists, from 9,103 to 8,981 specialists (-1.04%), and an increase in the number of ENT specialists working in non-state-owned MOs by 1,356 specialists, from 2,766 to 4,122 specialists (+33.0%).
Conclusions. The data from literary sources confirm the importance of studying and finding relevant solutions to the problem of staffing medical organizations in the healthcare system with otorhinolaryngologists, given that the prevalence of a group of ENT diseases does not show a downward trend, and the results of the analysis showed an imbalance in the availability of otorhinolaryngologists and different levels of their shortage in individual regions.
