EXPERT EVALUATION OF THE COMPETENCIES OF THE HEAD OF A REGIONAL MEDICAL ORGANIZATION: ANALYSIS OF THE DEGREE OF CONSENSUS

Summary:
Introduction. In global and national practice, an expert-based methods are often used to assess complex qualities that are not always amenable to formalization. The essence of this approach is the structured collection and analysis of the opinions of highly qualified specialists who have a deep professional understanding of the problem. Unlike quantitative methods, expert evaluation allows for the consideration of the nuances of professional activities, integrative abilities, and behavioral indicators of competencies that are difficult to measure. Purpose of the investigation. Based on the integration of regulatory requirements, literature data, and expert opinions, create a structured list of competencies for the position of heads of regional medical organizations and quantify the level of consensus among expert groups. Materials and methods. The inquiry was conducted in the Ryazan Region in 2024. Two groups of experts were involved in the development: heads of regional medical organizations (Group 1) and employees of scientific and educational organizations (Group 2), a total of 30 people. Statistical methods, the expert evaluation method, and the analytical method were used. Results and discussion. Experts from Group 1 demonstrate the highest degree of consensus in their assessments of two factors: «professional competencies based on the generalized labor functions of the occupational standards» and «competency assessment methods» (W, respectively, 0,95736 and 0,91983; p-value=0,03305 and 0,00054). For Group 2 experts, a very high level of compatibility was observed for the factor «generalized professional competencies» (W=0,907107; χ2=11,66667 with p-value=0,05093), which allows us to classify the competencies «administration of the activities of a medical organization and its structural units» and «quality management of the processes of a medical organization» as the points of greatest consolidation among this category of experts. The experts of the 2nd group unanimously concluded that the possession of meta-competencies is a prerequisite for the heads of medical organizations (W=0,76897; χ²=19,31034; p-value=0,04993), and express a high degree of confidence in the competency assessment methods (W=0,83010; χ²=58,43102). In general, the opinions on the competencies required for the official capacity of head of a medical organization are highly consolidated by both groups of experts (W=0,8326; χ2=86,03>11,07050; p-value=0,05133), and therefore, the hypothesis of expert consensus on the set of competencies for the capacity of head of a medical organization is accepted. The deviation of the excess from 0 is insignificant (ex=0,56093), which indicates a nearly symmetrical distribution of expert opinions (although with slight deviations). Conclusion. The study proved that the analysis of the list of universal and professional competencies required for the official capacity of head of a medical organization, based on expert evaluation , indicates a high level of consensus that has been confirmed mathematically. However, the degree of consensus is differentiated, as there are areas of relative disagreement in the assessments (the greatest range of views is observed regarding competency assessment methods).
