EXPERIENCE OF IMPLEMENTING NEW ORGANIZATIONAL MODEL OF NURSING STAFF IN THE CHILDREN'S DEPARTMENT OF THE CITY POLYCLINIC

Summary:
Improving outpatient care in pediatrics takes a leading place in the general health care system and is carried out through a wide network of pediatric polyclinics and polyclinic departments. The functioning of the pediatric service within the conventional polyclinic model doesn’t ensure rational use of health care resources. The shortcomings of the standard model over time have led to a number of problems in the outpatient care in pediatrics. The Order of the Ministry of Health of the Russian Federation No. 309 of June 25, 2014 "Action Plan to expand the functions of specialists with nursing education", which came into force, contains one of the key of development in nursing practice - independent role of a nurse or paramedic in the outpatient and polyclinic service. At the Assembly of the Russian Nurses Association, Mikhail Murashko, Minister of Health of the Russian Federation, once again reiterated the need to empower nursing staff. The purpose of this article was to analyze the results of the implementation of a new organizational model for the for the nursing staff in the pediatric district service in the outpatient pediatrics development. Research methods: In order to analyze patients' satisfaction with the quality of medical services in the conditions of the new organizational model, a questionnaire was conducted in which patients (parents or legal representatives) participated. During the analysis, the indicators were calculated according to a specially developed methodology. The photochronometric method of the study was used to conduct observations of the work of a district pediatrician. Results: the introduction of a new organizational model for the activities of the nursing staff in the children's department of the outpatient clinic allowed the district pediatrician to free up 9.8% of the time previously spent on processing medical documentation. Delegating some of the physician's functional responsibilities to district nurses allowed to redistribute the time of the district pediatrician's appointment and increase his primary activity by 10%. Self-administration by nurses in the pre-doctor's office promotes early active detection of patients with minimal clinical manifestations of chronic diseases and risk factors for their development. Partial replacement of the work of district pediatricians by district nurses within their professional competence allows to optimize the cost structure of the medical organization and increases economic efficiency by 2.05 times. 69.6% of patients are completely satisfied with the work of the pediatric area. Conclusions: the new organizational model increased the efficiency of the district pediatric service; expanded the authority of the district nurse; increased the availability of medical care for children by organizing the simultaneous work of the district pediatrician and the district nurse, who conducts independent appointments; expanded the scope of preventive work at the pediatric area; ensured the continuity of medical care by changing the structure of patient admissions, delegating monitoring of the patient condition.
